A couple of years ago, back when I was living in Iowa, there was a case
where a battered spouse kicked her husband out of the house; he threatened
to break back in and kill her.  She called 911, the cops were sent, but
before they could get there the SOB broke in.  At a range of ten feet, good
lighting conditions, very low stress (for a gunfight--she had a good three
or four minutes to calm herself, think her options through, and get mentally
prepared for battle before the SOB broke through), and even some training on
the part of the shooter (the woman had been hitting the range, just in case
this happened), the results were not very encouraging.  Of the six .357s
fired, the SOB took only one round, right in the meat of the thigh.  (This
was, fortunately, enough to stop him.)

Or look at Amadou Diallo.  When four highly-trained NYC cops mistook his
wallet for a gun and opened fire at point-blank range, only 19 of 41 rounds
hit--and these guys were highly trained members of an elite police unit.

Back when I was in Iowa, there was a bank robbery where one suspect took 15
of 17 rounds from an officer's Glock.  The range was contact--the Glock was
in contact with the perp's torso as the perp and cop wrestled for control of
the officer's gun, and even then, two rounds missed.  On another occasion,
three cops fired a total of 15 rounds at a suspect from a range of less than
10 feet.  The perp surrendered to the police--but it turns out not a single
round hit him.  (If memory serves, the cops were making an arrest at night
in the middle of an awful storm, though--so maybe they can get some slack
cut.)

The North Hollywood bank robbery comes to mind.  Col. Jeff Cooper has said
that the North Hollywood bank robbery could have been over in thirty seconds
flat, had there been a single deer hunter around with a .30-06; those guys
were standing still for part of their crime spree, and even with God-knows
how many cops shooting at them, there were no head shots.  Had the North
Hollywood robbery taken place in ME, one operative would have just broken
out a scoped .454 Cassull and dropped each perp with a single round to the
head from 40m (reduced to using the 10m template, of course, by using a 4x
scope).

If I could change just one thing about the ME combat system, it'd be that it
seems *too easy* to hit your target.  The real world records, as established
by criminals, civilians and police, just doesn't match up that well against
ME.
