Most modern LMGs can handle lots of rounds before they need a change, and
doctrine usually calls for barrel changes after several hundred rounds.  When
was the last time a character fired that much?  The US Army does not issue
spare barrels for the M249, SEALs do not carry spares for their M60E3s.  
Never had any major problems.

For most guns, I have malfunctions on 00 rolls, modified for muddy conditions
and the like.  Some low quality guns or rust buckets can go as low as 95-00.  
The good stuff is not going to malfunction unless something is wrong.  For
most folks the easiest way to resolve a malfunction is to roll a d6.  On a
1-4, it will take that many turns to clear the malfunction, roll of 5 the gun
is inoperable until disassembled, and on a 6 something has broken and needs
to be replaced.
